Under Eye Bags or Circles: What Causes Them?

Dark circles under the eyes are common with aging, because the skin of the lower eyelids becomes very thin allowing the underlying blood vessels, and the blood they carry, to be seen more clearly than in other places on your body. Most of the other skin on your body is much thicker, 2 mm, rather than 0.5 mm around the eye. There are several other potential factors that may also contribute to the appearance of under eye circles. Sometimes medications, as well as some medical conditions, can cause dark circles under the eye. Inadequate sleep and exhaustion can lead to dark under eye circles. Genetics can also play a role in the unwelcome look of dark circles or under eye bags.

Commonly, with dark circles under the eye, you may also have puffiness, which is often referred to as “bags under the eye”. In younger people, this can be caused by too much salt, fluid buildup, allergies or illness. As you age, puffiness under the eyes is typically due to bulging fat from the eye socket into the lower eyelid, in combination with progressive laxity of the skin and muscle of the lower eyelid. In addition, progressive fat loss in the upper cheek that occurs with age can lead to a depressed area along the lower rim of the eye socket that creates a curved valley under the lower eyelid, called the “tear trough”. Shadowing from the puffy eyelid into the tear trough may cause dark circles to appear even darker.

What Can Be Done?
We have a procedure called Lower Blepharoplasty to remove the skin, muscle, and fat around the lower eye that has become droopy. This can give a more youthful appearance to the eye.

How is Lower Blepharoplasty Accomplished?
If only fat removal from the lower eyelid is needed, an incision is made on the inside of the eyelid that will not be visible. If removal of excess skin and muscle is also needed, an incision is made. With or without light sedation, a local anesthesia is used. The procedure involves about 45 minutes. Within one to two weeks signs of bruising and swelling are resolved. Most people return to their normal level of activity within a week.
